We need to purchase an overlay for the UB04 and the 1500. In the past, we always purchased our overlays for checks and UB's from Electronic Laser Forms which then became VMP and then was bought by Wolters Kluwer Financial Services. In October, we received a letter saying that as of April 20, 2007 they will no longer be licensing and supporting healthcare forms and related products and services. I know that FormFast can provide this service, but the price I got from them seems a little steep to me so before I just blindly sign on the dotted line I thought I would ask the rest of you who you use for your UB04 forms overlay and if you would not mind sharing the cost with me as well it would give me a good basis for comparison.
