Yes, we have!
In fact we went live on Monday, and it was brought to my attention on
Tuesday. I opened a task with Meditech, so hopefully they will resolve the
problem.
It is definitely causing lots of confusion here.

[MEDITECH-L] ADM: Two Same Accounts

Has anyone had this problem in 5.6 Magic where 2 same accounts are
showing on the look-up list? We have 2 patients having this problem when
you do a look-up in ADM, NUR and OE but if I go to PHA, it shows only 1
account. According to MEDITECH, they don't have a fix yet. Have you
encountered any other problems relating to this issue (ie. double
charging, documentation,...)?
