Hi Maria, We did not add Diagnosis but we are using suggested problem per query link and it is the RN's responsibility to go in and initialize the care plan on the patients after the history and initial assessment have been completed. The LVN can go in after the initial plan of care has been developed and accept any additional problems or deny additional problems that the system has generated. Hope this helps. Susan Ottmers
