A check, payable to CSMFO should be forwarded along with your submittal to be received by CSMFO on the following schedule: EARLY BIRD SPECIAL - RECEIVED BY SEPT 1, 2000 NO FEE RECEIVED SEPT 2 THROUGH SEPTEMBER 30 $ 25.00 RECEIVED OCTOBER 1 THROUGH NOVEMBER 3 $ 50.00 Submit one fee for each category you apply to. No submittals will be accepted after the November 3, 2000 deadline. There will be no exceptions. Postmark dates do not qualify as received dates.
