I remember the spot on the form and thinking "hell I dunno" so I put something, 
I know the month after all. For the last 20 years I've just sent my mother a 
birthday card the first week of December and called her at the end of the 
second week, its somewhere in there.

Amex is allowing me to use her Maiden name now...

Wow, that is a coincidence!
A couple days ago, I had exactly the same experience with Amex, for exactly
the same reason.
But how did THEY know my mother's dob?
