The 392 would probably get double digit mileage if I stayed out of the go pedal 
but I bet it'll be difficult to do that...

They're both relatively easy to find parts for so I guess it'd be a matter of 
what the truck came with...

Long as you are dreaming....  get the IH392 engine with 4bbl.  It 
will get as "good" mileage as the 345, but is a lot more "fun"
