Who signed off on those drawings? Wile E. Coyote (Sooper Genius) perchance?

We are working on a case right now where the engineers and the architects both had their heads up their respective asses. The City of Wichita passed a bond to rebuild/renovate some schools. One of the schools was built in 1928. The architect drew up plans to expand the school and add a whole other wing. In order to level the floors when the new wing was added to the old, they had to build up the concrete floor almost 8 inches. Rather than using foam or some other lighter method, they poured 8 inches of concrete on three levels/floors. As you might imagine three levels of 8 inches of concrete weighs a lot. A few years later, the elementary school starting collapsing. Luckily, the kids were moved out of the old portion of the school before one of the first floor walls collapsed into a classroom.

The project had contractors, architects, a team of engineers, and other experts and no one thought this was a bad idea.
