Water does not come into 100% contact with your head or radiator because of surface tension and Water Wetter works by reducing the suface tension of the water in your cooling system. The result is better thermal transfer from your head to the water in your cooling system and better thermal transfer between the water in your cooling system and your radiator cooling fins. If you'll look at the chart on the side of their can, you'll note that the product only helps a small amount.
You can do the same thing by adding a teaspoon of detergent to your cooling system.
