Not sure how'd you work a 4,6,9 shift? The working RPM range of a
American style big Diesel is ~1400-2100, every gear in a crash box is
about 500rpm, so skipping from 6 to 9 would mean you either rev the guts
out of the engine or lug it up from about 800rpm, neither of which is
good practice.
I used to shift 2,4,6,8 and then go to 9 at lower revs, there are also
driver who will shift every gear, loaded or not, guess they like the
noise the truck makes or something.
Far as manual trannies behind smaller turbo Diesels, the same concept
applies as bigger engines, once the revs drop below 1400 the turbo is
not as effective, to do with exhaust flow and volume. So if you got a
pretty high first gear it will be sluggish.

Heh. It's relative. Compared to with a trailer, its _fast_!
Compared to a typical car, the truck is still not terribly
speedy.
Most drivers don't seem to grasp the concept of _major_
skip shifting. Which is the fastest way to accelerate when
driving bobtail. 4,6,9 worked well for the 9-speeds, as I
recall.
