Diode packs CAN fail. The VR also has the brushes which wear, and it has a spring/leaf connection to the diode pack. That can develop corrosion with ... interesting... results. Remove the VR and check the brush length, as well as cleaning the metal contact finger (and the one inside the alternator too!) -j.
