BTDT. Easy enough to check, just drop the driveshaft. If the bearing and carrier don't spin with no vibration at all, it's bad.
You will have to separate the shaft halves to replace it. MARK THE DRIVESHAFT HALVES!!! It's not keyed, and if you don't put it back together on exactly the same spline, you will trade a roaring noise for the driveshaft shaking. DO NOT succumb to the temptation to replace the u-joint either, I never got mine to quit hammering!
