QTP setting already set to run on all rows

My datasheet has values in 3 rows for first column.

now how can i tell QTP to take data from action or global sheet?

Normally for recorded script, i used to parametrize this step in keyword 
view, but my keyword view shows a Statement for setting FORM name 
and i cant parametrize that.
