check dummy program and set default program format
According to the spec, a name returned by GenProgramsARB, but not yet bound, is
not the name of a program object, but Mesa binds a dummy program to this name.
So IsProgramARB should check this program. _mesa_delete_program(in
shader/program.c) should also check whether the program is a dummy program.
This patch set the format of a program to GL_PROGRAM_FORMAT_ASCII_ARB by
default.
