Run Miami and MuGuardianAngel/MuForce with nothing else apart
from setpatch and hardware drivers, and see if the same hits

Looks legit although it's hard to say without more information. What is
Miami doing (or does it seem to be doing) when the hit occurs?

Based on the information you've provided, I don't believe this
second hit is a result of a Miami bug.
In this hit, Miami is trying to allocate some memory but GA
discovers that part of it's mungwall has been overwritten.
Overwriting of the cookie could have been caused by any program -
the fact that it is Miami that is running (and hence reported by
GA) when the allocation occurs, tells us nothing.
GA should report whenever one of it's cookies is overwritten in
this manner. I therefore suspect there are other hit inducing
programs on your system.
