ISO 3166-1 [1] defines two and three letter country codes. As far as I
have seen, two letter codes seem to be the mostly widely used; for
example, in URI domain names.
I don't think there's a world-wide accepted state code system and the
whole concept gets somewhat tricky at times anyway. My practice has been
to use the postal abbreviations as defined by the country authority.
FIPS defines a worldwide set of region codes [2] that you may find of
personal interest.

I'm planning to put together some tools to give to event promoters to easily
add hcalendar markup to their events pages and data feeds on their own
websites and some kind of simple but flexible parser for hcalendar events to
be used by events listings sites like mine (spraci.com) to extract such
events from feeds submitted to them so that those events can be listed
without having to be entered again.
I don't think it would be reasonable to expect everyone to enter
latitude/longitude for every event to make sure their entry is not
ambiguous, not everyone has a GPS device and entering coordinates from a
list of general city/state/country lat/long coordinates does not seem ideal
because they would not be the actual coordinates of the venue (someone might
end up putting them into a mapping site and getting incorrect directions!)
