Please look at cite-rel - http://microformats.org/wiki/cite-rel - this
microformat was mainly designed to mark up distributed discussions but using
fragment references (#commentId) it can be used locally as well.

I don't see any real difference between the two. Using lists of lists (for
example XOXO lists) you can mark up hierarchical (and obviously, linear)
structures quite easily. This assumes that all comments are hosted in one
location. To make the relationship more explicit use cite-rel.
