Looks good.  Two questions
1. Wondering if we can make the "Spouse/Father Name" a label that can be
renamed?
2. How will the dropdowns be populated?  Will that be configurable?

Just thinking others might find this useful to re-purpose for things like
adding guarantors.

Fair enough, I've heard this comment more than a few times.  It would
probably take enhancing the custom fields feature which might be more work
than its worth at the moment.  Some day :)
