Bizarrely incorrect change log

We were trying to track down another problem with a loan and when we used the
change log feature we could see that it is incorrect. Instead of giving
information about the fields that were changed by the actual users that changed
them, it shows a user who we know did not log-in that day, and shows fields
that are not mutable, such as global_account_num and accountId. Another
strange thing is it says the account balance was changed from 1100 to 500
(twice) but you can see in the loan detail that it is still 1100. (see
screenshots)
Unfortunately we do not know how to repro this. We didn't do anything unusual
(i.e. anything that was not done through the mifos UI). We don't know how or
why this happened, or how often it may be happening to other loans. But we are
afraid that if the change log data is bad, we won't be able to figure out some
other problems.
So, maybe we can help get some clues from the database where this occurred?
What should we look for?
