I'm re-opening this issue and closing the other. THe other bug specifically
said that custom fields should display in the hidden/mandatory screen. That's
not how we want to support this scenario. Insted, in the "Additional Fields"
UI, we should have the ability to remove/hide previously defined custom fields.
