Changes to XML upload UI

The following changes are needed to the already implemented XML upload UI.
1. Modify the link 'Upload PPI Question Group' to 'Activate Question Group'.
2. The link should be moved under 'Data display and rules'.
3. On click of the above link, the label country should be replaced with
'Select Question Group'.
4. The text 'Upload Progress out of Poverty Index Question Group' should be
changed to 'Activate Question Group'
5. The text 'Select the country and click Submit' should be changed to 'Select
the Question Group and click Submit'.
