To best facilitate translation, this should be a single phrase. Context is
everything for translators, and phrases should stand on their own as much as
possible.
Investigate and implement strings in such a way that only complete phrases are
present in the message bundles. One idea (not sure if this works or not):
questionnaire.create.question=Click on a question below to view and make
changes or {0}define a new question{1}

{noformat}
0=<a href="/new">
1=</a>
{noformat}
or something like that.
