PPI Dump Report: improve date formats

Exported PPI data dump report shows dates initially in excel with value
"00:00.0".
After double-clicking date on the date fields on the attached csv, the date now
appears as "6/19/2009 0:00"
Two issues:
1. provide formatting so date shows up immediately in cell for
date_survey_taken, start_date, and date_of_birth.
2. don't show hours, minutes. Only show date.
(Kay may have clarification on how to display dates) propose YYYY-MM-DD
