'Add new question' radio button is not working on 'Add Question Group' page

Currently, 'Add new question' radio button on 'Add Question Group' page is not
working properly.
Repro:
1. Login to Mifos.
2. Go to Admin page.
3. Click on 'Add New Question Group' link.
4. On 'Add Question Group' page, select 'Add new question' radio button.
Expected result:
Form for adding a new question appears.
Actual result:
Nothing happens.
