It seems that you have some duplicated entries in the database.
Unfortunately it won't be possible for us to diagnose and fix the
problem by looking into the stack trace only.
Is it possible for you to send me a drop of the database? Then I
will look into it.
If so, I'll need these things to reproduce your issue:
-drop of your database
-AplicationConfiguration.custom.properties file
-A screenshot of your system settings form mifos (Admin -> View
System Information)
