If you tried connecting by manually performing an SMTP conversation, be sure to connect from a constant IP address and be especially careful to send exactly the same information for the MAIL FROM and RCPT TO commands. A simple typo can mess up your test and explain your problem. To prevent typing mistakes, you may want to consider scripting a test, e.g. by using nc(1) and a constant SMTP conversation.
