My first reaction: Looks like a bad media, a bad drive, or a bad burner used to create the disk. Do you have any reason to believe otherwise? Looks like a SATA DVD ROM drive attached to an ahci controller, which is kinda new stuff (at least for me! :), so I'm not going to say there ISN'T an OpenBSD problem, at least until I get some better HW in production here to try it, but I'd certainly start with the obvious.
