I get similar problems on my computer after a few weeks of uptime. Where it's swapping continually and I can't even log in on the console. I only have 32MB of RAM so I think the effects are more pronounced for me. I also have some output from ddb if that would be helpful. I'll post a dmesg later but I'm running a snapshot from early May I think, and have been seeing this problem for a few months.
