The reason for having all these problem was the inappropriate use of
'auth' in the smtpd.conf file.

Below, as reference, the 2 configuration files:

smtpd.conf

listen on $egress_int pki server.pki tls-require hostname [domain] mask-source
--------------------------------------------------^ NO auth!!!
listen on $egress_int pki server.pki smtps auth hostname [domain] mask-source

config.inc.php

$config['default_host'] = '127.0.0.1';
$config['smtp_server'] = '127.0.0.1';
$config['smtp_user'] = '';
$config['smtp_pass'] = '';
