Can you come up with a sequence of steps that can reproduce this
starting from an empty database?

Insert image same image again.
Now image is displayed, Card looks like this

PS
At first it was worse; seems like many of the pictures were erased from
.db_media. I copy-pasted from back-up. At first that seemd to solve problem
