Completed account lineage/ancestors retrieval in DAO layer.
- Reviewed core algorithm and overloaded lineage retrieval using both account
and accountSid.
- Added and reviewed tests and sample data.

Progress on three-level account support.
- Adapted access control rules to use all ancestor accounts instead of only
parent.
- Cleanup of SecuredEndpoint.
- Central error handling for too-many-account-levels error.
- Support for lazy retrieval of effective account lineage in
userIdentityContext.
- Prevent account creation beyond third level.
