As of mod_perl 1.20, pnotes was formally documented.  I recall the
discussion between Doug and others being that Doug was unsure if he was
going to keep it, but the community loved it so it made it into the official
API. The dialogue is in the archives somewhere, and the addition documented
in mod_perl source Changes.
