Thanks for everyone's responses. I managed to get this working in principle by:

With mod_perl 2, you can use http://search.cpan.org/dist/Apache-Filter-HTTPHeadersFixup/ which is perfect for proxying as it does the work long before mod_rewrite kicks in.
