Ugh, coffee hasn't kicked in yet here :)

Looks like it should be CPPFLAGS instead of CFLAGS [1]. If that
doesn't work, you can edit the generated Makefile by hand and add
/usr/include/apache2/modules/perl to the include path for compilation.

Are you using the OpenSUSE mod_perl install?

[1]
ph...@pooky ~/dev/svn/sl/trunk/SL-App/src/libapreq2-2.12 $ ./configure
--help | grep include
      --help=recursive display the short help of all the included packages
  --includedir=DIR       C header files [PREFIX/include]
  --oldincludedir=DIR    C header files for non-gcc [/usr/include]
  --disable-FEATURE do not include FEATURE (same as --enable-FEATURE=no)
  --enable-FEATURE[=ARG]  include FEATURE [ARG=yes]
  --with-tags[=TAGS]      include additional configurations [automatic]
  CPPFLAGS    C/C++/Objective C preprocessor flags, e.g. -I<include dir> if
              you have headers in a nonstandard directory <include dir>
