I have server wide set to Extreme for spam with almost no false positives.
I've told people to check their quarantine reports often and whitelist stuff
that they want to receive.  I have a few domains that elected to use just
Strong for the spam filter (two of them changed their mind after a few days)

We're still using the text file.  I'd prefer to reject certain connections
outright rather than wasting time and processing running them through the
spam filtering.  It's a limited text file with some of the real obvious spam
text strings and sources listed.
