I have been successfully managing Qmail with Monit for more than three years. My Web page explains how, but unfortunately most pages are only in Japanese. So I'm goig to introduce the link URLs to sample files;
Essensially I run Qmail components via tcpserver. They are start/stoped
through rc script I crafted. Each rc scripts, of course, create PID file.
Then you can setup Monit control blocks (attached to this mail) as usual.

My qmail-smtpd, qmail-pop3d rc scripts require a tiny shell script,
detectpid, which detects PID of invoked tcpservers without fail.
