Without further investigation I would assume this was related to the thread handle issue reported a number of times in the past. One of the bug reports is here:
http://bugzilla.ximian.com/show_bug.cgi?id=79060 We have resorted to restarting our server (and cleaning up the wapi files) nightly to work around the bug.
