domref update

Working on distinguishing the interfaces on the document object a little better per some suggestions from Garrett Smith.

Just checked in a quick fix to the overview stuff at http://www.mozilla.org/docs/dom/domref/dom_doc_ref.html in the meantime.

Also fixed the title on the TOC page.
