That's what I was thinking. We indeed need this functionality on a per-top-level-window level. It's not immediately clear to me though whether this impl applies to each top-level window. If it does, we need to push this out through either nsIWebBrowser, or maybe through nsIAccessibility (TBDiscusssed tomorrow).
