In new-toolkit apps, you rarely need to watch for profile startup, but if you do, you can register as an app-startup listener and then observe the profile-after-change notification on the global observer service.

I am building a component over Mozilla 1.6 and already nsQEngineImpl implements nsIProfileStartupListener.
