Thanks to all repliers. I think part of my problem might be due to my maintenance level (CSD5). We haven't had any problems up until now and even then it's a rare issue. I'm going to try to review what's been fixed since CSD5. Upgrading to V6 is in the plans, so no CSDs will be applied.
