Assuming you have an SNMP daemon of some sort running (e.g UCD-SNMP, NET-SNMP, etc) then it's as simple as pointing MRTG at it. It sounds like you don't have it running as you mention you see "Connection refused" messages. Have a look at "http://www.net-snmp.org", I use this without any hassles on Redhat Linux (works on a variety of *nix). Another thing to make sure is that the host your trying to query is not firewalling your requests (make sure you permit UDP port 161).
