Hi All,
I'm trying to create a data entry form where one dept can input data into a form based on a table (1) AND as they are certain fields are populated from data in other tables (2-5).  I know this can be done but I'm having a hard time finding the correct statement to put into the control source. 

I know this is probably easy stuff for you guys and I really have been spinning my wheels this morning so your assistance is GREATLY appreciated.
