I think it sounds like a UNION query.
 
something like....
 
SELECT ErrorBy1, [field2], [field3]...
FROM Table1
UNION SELECT ErrorBy2, [field2], [field3]...
FROM Table1
 
You need to know that a UNION query cannot be displayed in "Design" view
of the Query.  You must use SQL view.
