Details:
Internal links should only use the anchor id as the target.
It should be: <ulink url="example">text</ulink>
Not: <ulink url="QuickStart.xml#example">text</ulink>
Basically, the "url" attribute of <ulink> needs to be exactly the same as the
"id" attribute of <anchor>
