I've had problems with this as well. It seems to be caused by having
mailboxes in your muttrc that don't exist on the server.
From your debug log:
4< a1344 NO FT/com doesn't exist.
tls_socket_read (A TLS packet with unexpected length was received.)

tls_socket_read (A TLS packet with unexpected length was received.)
I.e, this is a bug in your configurations as well as a bug in mutt or
Exchange (I'm working against Exchange 2007).
