This works for me. What are you expecting to happen?
$MAILDIR in the environment sets the default value of $spoolfile within
mutt before .muttrc is opened, but it's not the only way that the
default value can be set. $MAIL in the environment supercedes MAILDIR.
Do you have $MAIL set to another path? Do you have $spoolfile set within
.muttrc?
