Hi Gregor,
Some things I would suggest to look at are:
1. The $metoo and alternates settings
(http://dev.mutt.org/doc/manual.html#alternates).
2. If the original email had a Reply-To header and your $reply_to setting.
3. If the original email had a Mail-Followup-To header and your
$honor_followup_to setting.
Keep in mind that alternates is a regular expression, so if you aren't
careful it may match more than you intended:

# good:
alternates '^foo@bar\.baz$"

[As an aside, it would be greatly appreciated if you would email mutt-
users or visit the #mutt irc channel to double check before filing a bug
for the developers for something like this. I have pretty high confidence
the group-reply command (albeit extremely customizable) is working
properly, and the mutt-users and #mutt folks are much better at helping
troubleshoot these kinds of things.]
