happens to me in various versions of vim (and in nvi as well i think),
on debian and on freebsd, using xterm, xterm-color (sorry thomas) or
xterm-xfree86 as $TERM usually and using Eterm or xterm generally as a
terminal emulator.  there's something about it in the vim faq but i've
never been able to figure out the best way to remap them (or why it
doesn't happen in insert mode).
