It was smtp_url. The definition in the mutt manual says
smtp[s]://[user[:pass]@]host[:port]
and I guess I got it confused with http://user:pass@domain, so had
smtp://me:p...@corp.com@smtp.office365.com:port
but it should be
smtp://m...@corp.com:p...@smtp.office365.com:port
