Yes, they did. As Martin mentioned, newer HOWTOs on the wiki cover the 
additional steps required for the newer H3 hardware.

(We're planning to reorganize the HOWTOs and have the main mvpmc.org 
site point to the wiki, rather than the old HOWTOs it currently points to.)

Yes, the H3s broadcast a request that is answered by a special "glue" 
server, like mvpserver or mvpboot, which replies with the location of 
the TFTP and media servers.

Do you have a TFTP server running on the standard port 69 in addition to 
the high numbered port? While the Hauppauge firmware uses the high 
numbered port to load the dongle, the mvpmc startup code still uses the 
standard TFTP port to load the config file.

I'd recommend setting up your TFTP server so it is logging file requests 
(which it may do by default) and watch its log during a boot.

That actually sounds like it is succeeding in loading the config file, 
but the config file has an error. If the mvpmc startup code fails to 
load a config file, it normally starts the mvpmc software for you. In 
many cases, there's no need for a config file.

Given that the config file is actually executed by the MVP, it tends to 
be error prone, and a simple typo will prevent mvpmc from starting.

What I gather from messages I've read on the developer list is that 
these two aren't necessarily related, but there was a change introduced 
that caused problems with live TV and there is a patched dongle floating 
around that one of the developers created which fixes it. I think last I 
read they were trying to determine why the patch fixed the problem and 
considering incorporating it into the main code. Check the developer 
list archives.
